Abstract as Published:

In any problem solving journey, failure analysis (FA) is key in identifying the root cause of any failure. Diagnosis given by FA provides insights on mechanisms by which failures occur. This helps in determining factors that lead to the failure. With those factors identified, it would be easier to pinpoint the root cause and consequently provide corrective actions. In this paper, we focused on a specific type of Protective Overcoat Damage (POD) whereby the FA team used several approaches, techniques and tools to characterize the physical failure. Researches and collaborations with worldwide counterparts, resource people and international experts were also pursued to further understand the mechanism of the defect. The complexity of the physical failure, suggested different root causes. This triggered the formation of a cross functional team, which with FA had identified wash water particulates less than 0.2um punctures protective overcoat at higher water pressure rate of 75-80psi as the root cause. This paper details the investigation flow and findings as well as corrective and preventive actions generated to resolve and prevent recurrence of the problem.
